REPORT AN INCIDENT OR COMPLAINT
Wynnum Wolves Football Club
Choose the form that fits your concern. If you are unsure whether a child-safety concern is involved, use the Child-Safety & Conduct form.
If a child or any person is in immediate danger, call 000. If the concern may involve a criminal offence, contact Queensland Police. If immediate assistance is required at a WWFC venue, contact the Sports Trainer / Match Day Official on duty. Submitting a form does not replace any other reporting obligation.
Step 1: What would you like to report?
Online Child-Safety & Conduct Reporting Form
For concerns involving a child or young person, or the conduct of a worker/volunteer towards a child.
You do not need to decide whether the matter is “reportable conduct”. Tell us what happened. WWFC will assess the concern, take any immediate safety action required, and determine whether investigation and/or external reporting is required.
Online Incident / Complaint Form
For incidents or complaints that do not involve child-safety concerns.
Is a child-safety concern involved? If this matter involves the safety or wellbeing of a child or young person, or the conduct of a worker/volunteer towards a child, please use the Child-Safety & Conduct Concern tab above instead. If you are unsure, use the child-safety form.
Important
These are WWFC internal intake forms. Completing a form does not itself constitute any statutory notification that may be required to Queensland Police, Child Safety, the Queensland Family and Child Commission, Blue Card Services, Football Queensland or another regulator / authority. WWFC will assess the information received and determine its reporting and safeguarding obligations.
Reports are sent to Karl Dodd (CEO / Child Safety Officer) and Rabieh Krayem (Club President). If your report is about one of them, answer the last question in the form and it will only be sent to the other.
